Shoulder pain that flares up specifically at night — disrupting sleep and making it hard to find a comfortable position — is a common and frustrating complaint. Several distinct conditions tend to cause this particular pattern of pain.
Rotator Cuff Issues
Rotator cuff tendinopathy or tears are among the most common causes of night-specific shoulder pain. Lying on the affected shoulder compresses already irritated tendons, while lying on the opposite side can still cause pain due to gravity pulling on the shoulder structures.
Frozen Shoulder
Adhesive capsulitis, commonly known as frozen shoulder, often causes pain that’s notably worse at night, alongside progressive stiffness and reduced range of motion. This condition typically develops gradually over weeks to months.
Bursitis
Inflammation of the bursa — a small fluid-filled sac that cushions the shoulder joint — can cause sharp pain with certain movements and a deep ache that worsens when lying on the affected side.
Poor Sleep Position
Sleeping on your side, particularly on the affected shoulder, compresses the joint structures and can aggravate almost any underlying shoulder issue, even a relatively minor one.
Referred Pain from the Neck
Sometimes shoulder pain at night actually originates from the cervical spine, with nerve irritation or referred muscular pain presenting as shoulder discomfort, particularly when lying in certain positions.
Managing Night Pain
- Try sleeping on your back with a pillow supporting the affected arm.
- If side sleeping is unavoidable, use a pillow to support the arm and reduce joint compression.
- Apply gentle heat before bed to reduce muscular tension around the joint.
- Avoid overhead activities or heavy lifting in the hours before sleep.
